*Rates listed reflect base hourly or package rate. An additional travel rate may be added for the following:
| Virtual in any location in Indiana, North Carolina, or Virginia | No additional travel rate; base rate only |
| <45 miles from Crawfordsville, Indiana home office | No additional travel rate; base rate only |
| 46 miles or more from Crawfordsville | $0.50 per additional mile to and from (Examples: South Bend ~$45, Fort Wayne ~$97, Evansville $126) added to base rate |
Both virtual and in-person initial assessments (required to determine recommendations at the start of services) are a base rate of $110. Travel rates above may apply.
